CCF Leverages GSI Health Platform for Community-Based Care Programs
PHILADELPHIA, PA: According to a recent announcement from GSI Health, Collaborative Family and Children (CCF) has agreed on implementing the GSIHealthCoordinator, a care coordination and population health platform as their technology solution. CCF works as a health home solution serving more than 60,000 children in New York everyday whereas GSI Health’s cloud-based software solutions unite patients, health care payers and care providers of all kinds. The GSIHealthCoordinator platform comprises of a range of care coordination apps, population health analytics, and reporting for CCF's population health initiatives, including the state-led Children's Health Home, Medicaid Redesign Team Delivery System Reform, and more.
CCF has an outstanding range of services stretching across New York, covering schools and educational programs, Head Start and other early childhood services, clinics, case management, after-school and summer youth programs, and residential treatment services. Serving 80-85 percent of youth in foster care, as well as 60 percent of families in receiving preventive services is what constitutes a major chunk of their work. Collaboration across these diverse care institutions is complicated and the customizable, GSIHealthCoordinator platform is just the solution that integrates installations specifically configured for individual program.
