CCF Leverages GSI Health Platform for Community-Based Care Programs
PHILADELPHIA, PA: According to a recent announcement from GSI Health, Collaborative Family and Children (CCF) has agreed on implementing the GSIHealthCoordinator, a care coordination and population health platform as their technology solution. CCF works as a health home solution serving more than 60,000 children in New York everyday whereas GSI Health’s cloud-based software solutions unite patients, health care payers and care providers of all kinds. The GSIHealthCoordinator platform comprises of a range of care coordination apps, population health analytics, and reporting for CCF's population health initiatives, including the state-led Children's Health Home, Medicaid Redesign Team Delivery System Reform, and more.
CCF has an outstanding range of services stretching across New York, covering schools and educational programs, Head Start and other early childhood services, clinics, case management, after-school and summer youth programs, and residential treatment services. Serving 80-85 percent of youth in foster care, as well as 60 percent of families in receiving preventive services is what constitutes a major chunk of their work. Collaboration across these diverse care institutions is complicated and the customizable, GSIHealthCoordinator platform is just the solution that integrates installations specifically configured for individual program.
"Our challenge is to reduce healthcare costs while improving outcomes for children, many of whom are at risk," said Jodi Saitowitz, Interim Director, CCF. "GSI Health enables us to see beyond just medical data to the social and behavioral issues that affect this population - giving us insights that will allow us to proactively manage their needs."
GSI Health has a deep and extensive experience in all aspects of care coordination and population health management solutions, which supports health home, DSRIP, and other value-based programs. GSIHealthCoordinator will assist CCF in taking care of individuals and the whole population through integrated care teams spanning the entire care continuum. This is also highlighted by the array of services that caters across healthcare plan development, team collaboration, real-time alerts and enhanced risk stratification, and analytics.
"Our tight coupling of care coordination with analytics creates a seamless approach to population health management that helps our clients navigate the challenges of care management while dealing with the fast-changing regulatory landscape. We connect critical systems across the care continuum so CCF can effectively collaborate with its diverse network of providers to impact the lives of tens of thousands of children in New York," remarked Lee Jones, CEO, GSI Health.
