CivicHealth and CareWire Collaborate to Enhance Patient Care

FREMONT, CA:  CivicHealth, a provider of software for healthcare networks and human services organizations joins hands with CareWire, Inc., a mobile patient engagement platform provider to supplement CivicHealth’s connectivity solution with a mobile device.

CivicHealth’s platform works alongside EHRs and other IT systems to overcome the challenges of the industry by consolidating information from clinical and non-clinical sources. It facilitates execution of care management initiatives for focused populations thereby resulting in lower costs, increased efficiency and improved care.

CareWire enables providers to send personalized, precisely timed, procedure-specific instructions, education and reminders at the moment patients must do or know something relating to their care. Through this solution, the mobile device becomes an extension of care delivery to improve efficiency and effectiveness of patient preparedness, timeliness and adherence to treatment.

The capabilities will initially be deployed by the Indiana Rural Health Association to support and strengthen the association’s Rural Healthcare Care System and Primary Care-based Chronic Disease Management which is focused on helping patients manage chronic conditions including diabetes, congestive heart failure and chronic obstructive pulmonary disease.
