Humana and IntegraNet Partners to Provide Quality Care for Patients

Dallas, TX: Humana, a healthcare service provider, together with IntegraNet Physician Resource, an association of independent physicians inks a deal to create a new physician population health management partnership.

This deal offers personalized care and population management tools such as predictive analytics and chronic care, disease management and wellness programs. It also focuses on performance improvement, better patient care coordination and delivering cost effective quality care.

As per this collaboration, Humana offers Medicare Advantage Health Maintenance Organization (HMO) and Preferred Provider Organization (PPO) membership access to improve population health services by primary care physicians affiliated with IntegraNet.

HMO is an organization that provides managed care for health insurance, self-funded health care benefit plans, individuals and other entities. The plan provides integrated care and focus on prevention and wellness. In this plan, the patient can go to doctors, health care providers or hospitals on the featured list as well get specialist referrals for particular treatment from their respective primary care doctor. It also covers prescription drugs. On the other hand, PPO is managed care organization of medical doctors, hospitals and other health care providers that agree to provide health care for an insurer's or administrator's clients at reduced rates.

The partnership offers Humana members access to integrated care teams consisting of experienced physicians, nurses, health coaches and social services resources for providing high level care coordination with enhanced quality.

The alliances also leverages measures defined by National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS) such as diabetes care and treatment, breast cancer screenings, colorectal cancer screenings and high-risk medication.
