The Uniphy Care Platform creates a network to align the organizations’ acute and ambulatory care teams, skilled nursing facilities, home caregivers and patients to deliver high-value services at a lower cost and higher-quality. The platform also keeps patients engaged by sending secure messages to the care team in order to keep them informed about their regular clinical visits. Uniphy Care Platform ensures all members of the care team to collaborate with the management to offer special care for high-risk patients through High Risk Patient Management app.
The platform also offers Communications Health Assessment framework to determine how organizations deliver a positive return on investment (ROI) today and help those organizations competitiveness to value-based care for tomorrow. The full communication and collaboration suite includes acute care, physician engagement and home care solutions featuring secure text, VOIP calling, critical lab, consult delivery and on call schedules.
The platform also provides a double layer authentication for audit logs of messages, consults, receipts, administrator actions, data transfers and custody with 256-bit encryption. It is also designed to meet the HIPAA compliance and also warns the users, when they violate the regulations in order to promote clinical productivity.
