With an Eye on Triple Aim, Caradigm and Beacon Partners Join Forces
BELLEVUE, WA and WEYMOUTH, MA: Caradigm, a population health company, and Beacon Partners, a leading healthcare management consulting firm, have collaborated to develop a comprehensive solution that achieves Triple Aim. The solution is expected to provide enhanced population health management and yield value-based care.
This agreement would help healthcare providers in managing risk, care quality improvement with better care outcomes alongside reducing per capita cost of care.
Under the agreement, the companies will collaborate to help healthcare providers accelerate the deployment and adoption of Caradigm’s portfolio of healthcare analytics and population health solutions, including Caradigm Risk Management and Caradigm Care Management. As a critical part of this effort, Beacon Partners will offer advisory services, clinical and IT transformation, and implementation services for Caradigm customers. These services are designed to help organizations understand value-based care opportunities, assess their readiness to shift successfully to risk-bearing payment models, and develop comprehensive transition and implementation plans.
“Succeeding in population health and value-based care requires significant transformation on the part of providers, who are increasingly taking on more risk,” said Michael Simpson, CEO of Caradigm. “Beacon Partners brings proven experience in helping organizations navigate these changes. Our customers will benefit from Beacon Partners’ expertise in implementing new models of care delivery.”
“Healthcare organizations that will thrive in the future are those that successfully manage risk and improve the health of their communities,” said Ralph Fargnoli, president and CEO of Beacon Partners. “To do this, organizations need to leverage the full power of their data to transform their clinical, operational and patient engagement processes. We look forward to working with Caradigm to help clients navigate and gain value from these transitions.”
Caradigm’s family of software — anchored by the Caradigm Intelligence Platform — is designed to address the capabilities needed by healthcare organizations to improve population health management, including data aggregation and control, healthcare analytics, care coordination and management, and patient engagement and wellness.
